Forms for ICAA Programs. The Open Show Point Program (OSPP, $15 annually) and Trail Logging Program ($5 one time fee) Enrollment forms have the rules with them. The ICAA Distance Riding Program ($15 annually) has a separate sheet with rules. The OSPP Brochure is for your convenience if you want to print the front and back to form a folded brochure to hand out.
The Enrollment forms are fillable in their entirety and may be sent via email as an attachment and paid for online. The Reports are partially fillable. The Reports can be scanned when completed and sent via email as an attachment as well.
Other forms pertaining to registration. These forms are fillable, however most require an original signature, notary, and/or for the original Registration Certificate to be mailed with them via snail mail so the signature line(s) have been left blank and cannot be signed by digital signature. The only form here that can have a digital signature is the Signature Authorization Termination form.
A Stallion Breeding Report is required for all stallions that have covered any Appaloosa or other breed mares. Stallion Reports are due by December 31st of the breeding year and filing is free to current members when submitted on time. Form is fillable and may be sent via email as an attachment with digital signature and from an email address we have on file.
Required forms when registering a horse with ICAA are the Registration Application and the Pedigree Worksheet. An Extended Pedigree is required to calculate the horse's registry classification. Every horse in the pedigree must have their registry and registration number listed or show unregistered or unknown. Pedigree should be at least through the last full generation of registered Appaloosas, and the one preceding that to show the horse(s) stopping the registry classification from going higher. Our Pedigree Worksheet may be used or an extended pedigree printed out from the ApHC's website. No AllBreedPedigree or others like it will be accepted. We now have horses that go further than these five-generation forms/pedigrees go so extended pedigrees may be required on sires and dams or grandparents to cover them. NOTE: If the sire and dam are ICAA registered, these extended pedigrees are not required. If there are any ICAA registered horses in a horse's extended pedigree, the pedigree does not need to go further back on those horses.
Registration Application and Pedigree Worksheet are fillable and may be submitted via email. Registration fees may be paid online.
Membership is $15 and runs from January 1st to December 31st. Applications sent in from October to December will be good when received but applied to the following year. This is a bonus for new members. This form is fillable in its entirety and can be sent via email. Membership fee may be paid online.
The following official ICAA forms are provided for your convenience. They are all .pdf fillable forms and can be filled in entirely or partially on your computer, saved to your computer and uploaded as attachments unless original signatures are required.
These forms require Adobe to download. If you download another free program by Adobe, Adobe Acrobat Reader DC, you can function much more effectively with these forms and use the digital signature feature. The link to download this free program is https://get.adobe.com/reader/
